RESUMO

Calcium carbide residue (CCR), a by-product of the acetylene industry, is generated at a rate of 136 million tonnes per year, posing significant environmental risks. This review examines the potential utilisation of CCR in soil stabilisation, focusing on its stabilisation mechanism, performance in improving mechanical properties, environmental safety, and sustainability. The aim is to identify future research directions for CCR-based stabilisation to promote its broader application, and to provide references for managing similar Ca-rich wastes. CCR-based materials demonstrate promising benefits in enhancing various soil properties, such as uniaxial strength, swelling properties, triaxial shear behaviour, compressibility, and dynamic responses, while also reducing the mobility of contaminants. Compared to conventional stabilisers, CCR-based materials exhibit comparable performance in soil improvement, environmental impact and safety, and economic feasibility. However, further research is required to delve deeper into stabilisation mechanisms, mechanical properties, and stability of contaminants for the soil treated with CCR-based materials under diverse conditions.

RESUMO

With limited healthcare resources, it is important to provide the right level and form of care. The aim of this study was to determine whether selected single-jaw orthognathic surgery in outpatient care (OPC) generates lower healthcare costs than in inpatient care (IPC). The costs of surgically assisted rapid maxillary expansion (SARME), Le Fort I osteotomy (LFI), and bilateral sagittal split osteotomy (BSSO) were calculated for 165 patients, 107 treated in OPC and 58 in IPC. Additionally, costs for revisits, emergency visits, emergency phone calls, re-operations, and plate removal during the first 12 months postoperatively were recorded. The total mean costs of the different operations including revisits, emergency visits, and phone calls were 34.2-48.8% lower in OPC than in IPC at 12 months postoperatively. Operation costs were lower for LFI in OPC (P = 0.009) and for SARME in IPC (P = 0.007). Anaesthesia costs were lower for LFI (P < 0.001) and BSSO (P < 0.001) in OPC, and there were fewer revisits (P = 0.001) and lower costs (P = 0.002) after LFI in OPC compared to IPC. This study showed that selected single-jaw orthognathic surgeries in outpatient care are associated with lower healthcare costs compared to inpatient care.

RESUMO

In this study, we employed a straightforward flame synthesis process to produce carbon soot containing carbon nano onions (CNOs) using easily accessible ghee oil as a precursor. The ghee oil, with a molecular composition rich in more than 50 carbon atoms, served as an effective source for generating CNOs. The synthesized CNO particles underwent comprehensive characterization through high-resolution transmission electron microscopy (HRTEM), energy dispersive X-ray spectroscopy (EDX), Fourier transform infrared spectroscopy (FTIR), and X-ray diffraction (XRD) analyses, providing a detailed account of their physicochemical properties. In addition, we explored the direct deposition of CNOs on carbon fiber (CF) surfaces for 5 and 10 min via a soot deposition process. The resulting freeze-fracture images obtained from scanning electron microscope (SEM) offered insights into the morphology of the CNO-deposited CF. Our study aims to shed light on the potential applications of CNOs, focusing on their characterization and the possible benefits they may offer in diverse fields, including but not limited to enhancing interfacial bonding in thermoplastic composites.

RESUMO

Abstract Maydis leaf blight, caused by Bipolaris maydis, is an important disease of maize crop in Khyber Pakhtunkhwa (KP) Pakistan. Fifteen isolates of the pathogen, collected across KP, were studied for variability based on phenotypic and molecular markers. Significant variability among the isolates was observed when assessed using phenotypic traits such as radial growth, spore concentration, fungicide sensitivity and virulence. The isolates were classified into six culture groups based on colour, texture and margins of the colony. Conidial morphology was also variable. These were either straight or slightly curved and light to dark brown in colour. Fungicide test showed significant variation in the degree of sensitivity against Carbendazim. Isolate Bm8 exhibited maximum radial growth on carbendazim spiked plates. Conversely, isolate Bm15 showed the lowest radial growth. Variations in virulence pattern of the isolates were evident when a susceptible maize variety Azam was inoculated with spores of B. maydis. Genetic variability amongst the isolates was also estimated by RAPD as well as sequencing of ITS region. The RAPD dendrogram grouped all the isolates into two major clusters. Average genetic distance ranged from 0.6% to 100%, indicating a diverse genetic gap among the isolates. Maximum genetic distance was found between isolates Bm9 and Bm10 as well as Bm2 and Bm8. Conversely, isolates Bm13 and Bm15 were at minimum genetic distance. Phylogenetic dendrogram based on sequencing of ITS region grouped all the isolates into a single major cluster. The clusters in both the dendrogram neither correlate to the geographical distribution nor to the morphological characteristics.

Cardiovascular diseases during pregnancy are rare but account for complications that pose risks to the mother as well as the child. In patients with fixed cardiac output due to stenotic valvular lesion(s), the physiological changes during pregnancy carry high risk of morbidity and mortality. Case Presentation: Our patient was diagnosed with severe mitral and aortic stenosis at her first antenatal visit at 24 weeks of gestation. She was also diagnosed with intrauterine growth restriction and was therefore planned to be operated on at a gestational age of 34 weeks. After careful selection of monitoring and anesthetic regime, the patient was managed without any intraoperative or postoperative complications. Clinical Discussion: This case reports how the anesthetists, obstetricians, and cardiac surgeons devised a well-designed plan to operate on a patient with a relatively rare disease manifestation. Our patient had coexisting severe stenotic lesions of both mitral and aortic valves and posed a clinical dilemma regarding the choice of anesthesia and perioperative management. Regardless of the anesthetic technique, goals for a patient with the combined valvular disease include maintenance of adequate preload, systemic vascular resistance, cardiac contractility, sinus rhythm and avoidance of tachycardia, bradycardia, aortocaval compression, and anesthetic or surgery-induced hemodynamic changes. Conclusion: The course of management would give clinicians an idea of how to manage a patient with combined stenotic valvular lesions for cesarean section, ensuring a smooth course and a safe postoperative period.

RESUMO

The paper critically analyzed different interfacial enhancing methods used in thermoplastic composites. Although the absence of cross-linked polymer chains and chemical bonds on solidification enables the thermoplastics to be remelted, it creates weak interfacial adhesion between fibre reinforcements and the thermoplastic matrix. The weak fibre-matrix interface bonding reduces the efficiency with which the applied load can be transferred between these composite constituents, causing the composite to fail prematurely. Their need for high-temperature processing, poor compatibility with other polymer matrices, and relatively high viscosity render thermoplastics challenging when used to manufacture composite laminates. Therefore, various methods, including nanoparticles, changing the polarity of the fibre surface by plasma etching, chemical treatment with ozone, or an oxidative attack at the fibre surface, have been applied to improve the fibre/matrix bonding in thermoplastic composites. The fabrication steps followed in these techniques, their progress in research, and the associated toughening mechanisms are comprehensively discussed in this paper. The effect of different fibre-matrix interfacial enhancement methods on the mechanical properties of thermoplastic composites is also deliberated.

RESUMO

BACKGROUND: Ensuring health equity, especially for vulnerable populations in less developed settings with poor health system is essential for the current and future global health threats. This study examined geographical variations of COVID-19 mortality and its association with population health characteristics, health care capacity in responding pandemic, and socio-economic characteristics across 514 districts in Indonesia. METHODS: This nationwide ecological study included aggregated data of COVID-19 cases and deaths from all 514 districts in Indonesia, recorded in the National COVID-19 Task Force database, during the first two years of the epidemic, from 1 March 2020 to 27 February 2022. The dependent variable was district-level COVID-19 mortality rate per 100,000 populations. The independent variables include district-level COVID-19 incidence rate, population health, health care capacity, and socio-demographics data from government official sources. We used multivariable ordinal logistic regression to examine factors associated with higher mortality rate. RESULTS: Of total 5,539,333 reported COVID-19 cases, 148,034 (2.7%) died, and 5,391,299 (97.4%) were recovered. The district-level mortality rate ranged from 0 to 284 deaths per 100,000 populations. The top five districts with the highest mortality rate were Balikpapan (284 deaths per 100,000 populations), Semarang (263), Madiun (254), Magelang (250), and Yogyakarta (247). A higher COVID-19 incidence (coefficient 1.64, 95% CI 1.22 to 1.75), a higher proportion of ≥ 60 years old population (coefficient 0.26, 95% CI 0.06 to 0.46), a higher prevalence of diabetes mellitus (coefficient 0.60, 95% CI 0.37 to 0.84), a lower prevalence of obesity (coefficient -0.32, 95% CI -0.56 to -0.08), a lower number of nurses per population (coefficient -0.27, 95% CI -0.50 to -0.04), a higher number of midwives per population (coefficient 0.32, 95% CI 0.13 to 0.50), and a higher expenditure (coefficient 0.34, 95% CI 0.10 to 0.57) was associated with a higher COVID-19 mortality rate. CONCLUSION: COVID-19 mortality rate in Indonesia was highly heterogeneous and associated with higher COVID-19 incidence, different prevalence of pre-existing comorbidity, healthcare capacity in responding the pandemic, and socio-economic characteristics. This study revealed the need of controlling both COVID-19 and those known comorbidities, health capacity strengthening, and better resource allocation to ensure optimal health outcomes for vulnerable population.

RESUMO

Orthognathic surgery is traditionally performed in inpatient care. The question is whether patient safety is maintained when orthognathic surgery is performed in outpatient care. This retrospective cohort study was conducted to investigate patient safety in selected single-jaw orthognathic surgeries performed in outpatient care compared to inpatient care. Postoperative infection, postoperative bleeding, postoperative pain, plate removal, and re-operation, as well as emergency visits/phone calls and postoperative admission during the first 12 months after surgery were recorded. Predictor variables were sex, age, smoking, general disease, antibiotics, operation type, and operation time. Of the 165 patients included, 58 were treated in inpatient care and 107 in outpatient care. No significant difference was found between the groups regarding postoperative bleeding, pain, plate removal, re-operation, or emergency visits/phone calls. Ninety-four percent of outpatients (n = 101) were able to leave the hospital on the day of surgery as planned. There was an increased risk of postoperative infection in the outpatient care group (odds ratio 2.46, P = 0.049). Selected single-jaw orthognathic surgery can be performed in the outpatient setting, with maintained patient safety. The reason for the increased risk of postoperative infection among patients operated in outpatient care should be investigated in further studies.

RESUMO

Abstract The poultry sector in Pakistan is contributing mainly in bridging gap between demand and supply for protein. Mycoplasma gallisepticum is an emerging bacterium causing serious problems in poultry industry of Pakistan. A cross-sectional study was conducted to evaluate the M. gallisepticum load in poultry populated regions of Pakistan. Total 600 serum and 600 swab samples were collected, 200 from each broiler, layers and breeders poultry in Rawalpindi and Abbottabad districts. Serum samples were analyzed through ELISA for seroprevalence. Swabs were cultured on Frey's medium followed by PCR and partial mgc2 gene sequencing. Results of seroprevalence of M. gallisepticum showed that layers (75%, n=150) are more positive as compared to breeders (70%, n=140) and broilers (50%, n=100). Typical colonies of the M. gallisepticum were observed in breeder (26.5%), followed by layer (21%) and broilers (9%). A total of 37.1% (n=42) samples were identified positive through PCR out of total 113 cultured based positive samples. A total of six M. gallisepticum isolates of current study showed 98-99 percent similarity with previously reported isolates on the basis of mgc2 gene partial sequencing. The M. gallisepticum was found highly prevalent in different poultry breads. Results of this study would add into basic data and provide a direction for livestock sector to strengthen a control strategy for mycoplasmosis in poultry farms.

RESUMO

Background and Aim: Transversus abdominis plane (TAP) block and local anesthetic infiltration (LAI) technique are used as part of the multimodal analgesic regimen after abdominal surgery. Postoperative opioid consumption and analgesic efficacy was compared using TAP and LAI techniques in patients undergoing gynecologic surgery in a randomized, controlled clinical trial. Material and Methods: Total of 135 patients scheduled for major gynecological surgeries were allocated into three groups: group T received bilateral TAP block with bupivacaine 0.25%; group I received LAI with 0.25% bupivacaine with epinephrine 5 µ/mL in the peritoneum and abdominal wall, and group C was control group. Anesthesia and postoperative analgesia were standardized. Outcome measures were cumulative and rescue tramadol consumption, numerical rating score (NRS) for pain and side effects in post-anesthesia care unit (PACU) at 4, 8, 12 hours postoperatively. Results: Tramadol consumption, need for rescue analgesia, and NRS for pain between three groups at 4, 8, and 12 hours postoperatively had no statistically significant difference (P < 0.05). In PACU, median tramadol consumption used for rescue analgesia between group T (15 (15-30)) and group C (30 (15-45)) (P = 0.035), and between group T (15 (15-30)) and group I (30 (15-52)) was statistically significant (P = 0.034). In PACU, the percentage of patients having NRS >4 on movement in group C (72%) compared to group T (46.5%) and group I (46.5%) was significant (P = 0.034). No statistically significant difference was observed in the incidence of side effects among study groups (P > 0.05). Conclusion: Except for the immediate postoperative period, neither TAP block nor LAI had added benefit to the multimodal analgesia regimen in patients undergoing gynecological surgeries.

RESUMO

Staphylococcus aureus is an important foodborne pathogen associated to food intoxication and other multiple infections in human being. Its presence in salted food is a serious issue due to its salt tolerance potential. A study was conducted to analyze the presence of enterotoxins producing drug resistance S. aureus in salted sea fish from Gwadar. Freshly persevered samples (n=50) of salted fish were subjected to analyze the presence of S. aureus using 16S rRNA and Nuc genes primers. The isolates were then evaluated for drug resistance and enterotoxins producing potential using specific primers for MecA (methicillin resistance gene), (SEA) staphylococcal enterotoxin A and (SEB) staphylococcal enterotoxin B genes. Total 13/50 (26%) of the samples were found positive for the presence of S. aureus, preliminary confirmed with biochemical profiling and finally with the help of target genes presence. The isolates were found showing 100% resistant to methicillin, which were molecularly confirmed by the presence of MecA gene present in genome. The isolates 5/13 (38%) were positive for SEA and 3/13 (23%) for SEB genes, whereas 2/13 (15%) were confirmed having both SEA and SEB genes in its genome. It was also confirmed that all the isolates were capable to form biofilm over the glass surfaces. It was concluded that the study confirmed the presence of enterotoxigenic methicillin resistance Staphylococcus aurous (MRSA) in salted fish product, that poses gross food safety concern. Preventive and control measures are necessary to handle this serious food safety concern.

Paraneoplastic syndromes (PNS) are uncommon, distinct clinical complications of a primary tumor. Paraneoplastic cerebellar degeneration (PCD) is a PNS that is described as an autoimmune response targeting Purkinje cells in the cerebellum. Ovarian cancer (OC) is one of the most prevalent causes of cancer-related deaths in women. Anti-Yo is the most common onconeural antibody produced in the PCD immune response and is most typically found in ovarian and breast cancer patients. While the current literature highlights the predisposing genetic factors, diagnostic workflows, and treatment options, the pathophysiology of PCD, among other considerations, remains largely unestablished. This review aimed to systematically observe procedural solutions to facilitate an early diagnosis and improve the prognosis of patients with OC-associated PCD. To that end, we examined literature published from 01/01/2015-11/10/2022 indexed in PubMed by using the keywords "paraneoplastic, cerebellar degeneration" combined with "ovarian cancer." Inclusion criteria were met if PCD and OC diagnoses were made and if studies provided adequate patient information. After screening and assessing records for eligibility using the inclusion and exclusion criteria, 18 articles involving 102 patients were included. The typical patient observed in this sample was diagnosed with International Federation of Gynecology and Obstetrics (FIGO) Stage III, high-grade serous carcinoma. The diagnostic workup typically included a clinical evaluation for dysarthria (50%), ataxia (60%), and gait abnormalities (50%), along with multiple imaging modalities and serological findings (90%). Genetic screening for human leukocyte antigen (HLA) haplotype susceptibility for PCD and immune tolerance modulators regulation may also be recommended prior to starting treatment. Findings support the use of corticosteroids (35%) and intravenous immunoglobulin (IVIg) (40%) as viable treatment options for managing PCD in conjunction with systemic therapy for the primary malignancy. A diagnosis of PCD should be considered if a patient has had a malignancy in the past five years with the presence of explicit cerebellar symptoms. This clinical diagnosis can be further supplemented by serologic and radiologic findings. Recognizing PCD symptoms and scheduling genetic and proteomic testing may help with early diagnosis and better prognosis.

RESUMO

This study investigated the improvement in the behaviour of a clay soil due to the addition of alkali-activated fly ash as a stabilising agent, and the effects of different activation factors such as alkali dosages and silica moduli. The alkali activator solution used was a mixture of sodium silicate and sodium hydroxide. Class F fly ash was used as the precursor material for the geopolymerisation process. Soil samples stabilised with non-activated class F fly ash were prepared and tested to compare the results with samples stabilised with alkali-activated fly ash. Compaction tests, unconfined compressive strength tests, X-ray diffraction analysis, and scanning electron microscopy analysis were carried out on samples cured 1, 7, and 28 days at room conditions. The results showed that the compressive strength of stabilised soil significantly increased when the fly ash was activated. The optimal activation parameters to stabilise the soil were found to be alkali dosages in the range of 12% to 16% and a silica modulus of 1.25. The highest compressive strength recorded was at 1293 kPa with an alkali dosage of 16% and a silica modulus of 1.25, while for the non-stabilised soil, it was at 204 kPa at 28 days of curing. Mineralogical analysis showed a decrease in the peak intensities of kaolinite and illite, while microstructural analysis indicated an alteration in soil texture with the addition of the alkali-activated fly ash.

RESUMO

IntroductionEnsuring health equity, especially for vulnerable populations in less developed settings with poor health system is essential for the current and future global health threats. This study examined the heterogeneity of COVID-19 mortality and its association with population health characteristics, health care capacity in responding pandemic, and socio-economic characteristics across 514 districts in Indonesia. MethodsThis nationwide ecological study included aggregated COVID-19 cases data from all 514 districts in Indonesia, recorded in the National COVID-19 Task Force database, during the first two years of the epidemic, from 1 March 2020 to 27 February 2022. We calculated incidence and mortality rate by time, sex, and age. We extracted district-level socio-demographics, population health, and health care capacity data from government official sources. We used multivariable linear regression to examine factors associated with higher mortality rate. ResultsOf total 5,539,333 reported cases, 148,034 (2{middle dot}7%) died, and 5,391,299 (97.4%) were recovered. The national mortality rate was 55 per 100,000 population, ranged from 13 per 100,000 population in Papua to 156 per 100,000 population in East Kalimantan province. At district-level, higher mortality rate was associated with higher COVID-19 incidence (p<0.0001), higher proportion of [≥]60 years old population (p<0.0001), higher prevalence of diabetes mellitus (p<0.0001), lower prevalence of obesity (p=0.019), lower number of doctors per population (p=0.001), higher life expectancy at birth (p=0.035), and lower formal education (p=0.021). There was no association between COVID-19 mortality rate with expenditure, prevalence of hypertension and pneumonia, vaccine coverage for [≥]60 years old population, number of nurses, midwives, and hospitals per population (p>0.05 each). ConclusionCOVID-19 mortality rate in Indonesia was highly heterogeneous and associated with different prevalence of pre-existing comorbidity, healthcare capacity in responding the pandemic, and socio-economic characteristics. This study revealed the need of health capacity strengthening and better resource allocation to ensure optimal health outcomes for vulnerable population. What is already known on this topicO_LIThe severity of COVID-19 illness and clinical outcomes can be affected by the concentration of comorbidities in susceptible groups in communities, and through disparities of access to health care for preventive measures or prompt diagnosis and treatment. C_LIO_LIHowever, evidence on the heterogeneity of COVID-19 impact from low- and middle-income country (LMIC) where differences in age distribution, comorbidities, access to quality health services, and other factors, may greatly influence mortality risk, are limited. C_LI What this study addsO_LIThis study affirmed that COVID-19 disproportionately affected areas with high proportion of elder population, high prevalence of diabetes mellitus, lower doctor to population ratio, higher life expectancy at birth, and lower level of formal education. C_LIO_LIThese findings indicate that vulnerability to death associated with COVID-19 in LMIC includes not only elder and comorbid, but also males and communities living in area with lower health care capacity and with lower level of education. C_LI How this study might affect research, practice and/or policyO_LIThese findings may inform decisions on health resource allocation against COVID-19 delivering the greatest possible health dividends by prioritising interventions, including even distribution of essential health care need such as doctors, and a tailored risk communication and community engagement for the most vulnerable communities in LMIC, especially with decentralised health systems like in Indonesia. RESUMO

A novel electrokinetic geosynthetic (EKG) can be efficient in achieving multiple objectives. In this study, a new EKG as an electrode and a drainage channel in the electro-osmotic consolidation was fabricated by electrospun nanofibers containing graphene nanoparticles (GNs) attached to a carbon fiber substrate. To investigate the effectiveness of the fabricated electrodes in electro-osmotic consolidation and remediation of water drained from the system, an experimental apparatus was constructed while considering loading capability in expanded ranges and applying the electric field, and was filled with copper (Cu)-contaminated kaolinite. Experiments were divided into control (CT) and EKG groups, and three categories, C-EK, ES1-EK, and ES2-EK (using carbon fiber, electrospun nanofibers containing 1 wt% GNs, and electrospun nanofibers consisting of 2 wt% GNs, respectively). All the experiments were conducted with the same conditions, loading, drainage condition, and duration. However, EKG experiments were performed by employing the electric field under the vertical pressure in the range of 7-113 kPa, while the CT was conducted without the electric field. According to experimental results, 18 wt% polymethyl methacrylate in the dimethylformamide solvent containing 1 and 2 wt% GNs was selected for making a nanofibrous layer on the carbon fiber. The average diameters of the fibers were 404 ± 36 and 690 ± 62 nm and yielded at 1 and 2 wt% GNs, respectively. The results revealed that using the EKG accelerated kaolinite consolidation. The average degree of consolidation was 68 and 85% in the CT and EKG experiments, respectively. Furthermore, the fabricated electrodes were highly effective as a drainage channel for remediating water drained from the system. Moreover, the highest Cu removal efficiency was obtained in ES2-EK (97%) and ES1-EK (92%), respectively. Conversely, the lowest Cu removal efficiency was observed in the C-EK group (85%).

RESUMO

A large amount of coal fly ash produced in thermal power plants is disposed of in landfills which causes many environmental problems. The utilization of fly ash can be encouraged in geotechnical engineering projects. In this paper, the effects of class C and class F fly ash on the mechanical and microstructural behavior and stabilization of clay soil were evaluated through a program of laboratory experiments. The experiments included compaction, unconfined compressive strength, consolidated-undrained triaxial, one-dimensional consolidation tests, and scanning electron microscopy analysis on samples of fly ash-stabilized clay soil after 1, 7, and 28 days of curing. The tests were conducted on mixtures of clay with class C or class F fly ash, ranging from 0% to 30% of the soil. Experimental results showed that the strength parameters and permeability of the stabilized soil improved while the compression and swelling indices decreased by the addition of fly ash and by the increase of curing days. The results obtained from the mechanical tests agreed with the results from the SEM analysis. Based on the results, the soil could be successfully stabilized by using class C fly ash. The improvements in strength, swelling, and permeability parameters of the stabilized soil were higher with the class C fly ash compared with class F fly ash.
